Your First Visit
We want to make your first visit a productive one. Here is how you can help.
- If needed, please obtain your referral authorization from your primary care or specialist physician prior to your first appointment.
- Have your insurance card with you and available for the receptionist to view. If you have a co-pay, please be prepared to pay this at the time of your visit.
- Complete all sections of the patient registration and health history forms, and bring the completed forms with you to your first appointment. You may download these forms here.
- Bring any x-ray or other imaging studies pertaining to your orthopedic condition to the office with you to your first appointment. This allows your provider to conduct a comprehensive evaluation of your condition.
- Notify us at least twenty-four hours in advance if you cannot keep your appointment.
- Payment is accepted in the form of cash, check, credit card, or debit card.
Your physician visit will start with a personal interview and physical examination, which may be followed by diagnostic tests such as X-rays or other tests. Your doctor may be able to discuss your diagnosis and available treatment options with you that day, or you may need to wait for test results. Either way, we will help you choose option that best fits your lifestyle.
